I am after some help with transcription of a military medical discharge record if anyone can assist. How is it best to send it to someone if they offer to help?

Is it a question of not being able to read what is written, or merely a case of not understanding abbreviations used?
If the latter, then transcribe the entries.
If the former, then follow the instructions here.
Do not try to scan the whole document because it will be too small to read but leave enough writing either side of the bit you can't read so there are some other words for comparison.

This is from a medical discharge document
I think I can make out the following:
This man partially dislocated the sternal end of right clavicle by a fall....appears to have...partially lacerated? at the head of the.....kept in its place.....is recommended to be sent home? to the ...to try the effects of time....his conduct.

Progressed by a medical friend to:
This man partially dislocated the sternal end of the right clavicle by a fall his anterior ligament appears to have been partially lacerated at the head of the bone cannot be kept in its place the slightest movement of the arm......is not at present fit for active service it is recommended to be sent home? to the ...to try the effects of time....his conduct.

An attempt:
This man partially dislocated the sternal end of right clavicle by a fall the anterior ligament appears to have been partially lacerated as the head of the bone cannot be kept in its place the slightest movement of the arm causing it to start(?) forward, as he is not at present fit for active service he is recommended to be sent home to the depot to try the effects of time in repairing the injury his ailment has not been aggravated by this or his conduct(?)
Short on punctuation but so is the original!
