Two of my husband's uncles were Flight Engineers in the RAF and both were shot down and killed. I have looked for them on the GRO Index and cannot find either of them. Does anyone have any ideas why their deaths would not have been registered?

Hello, and welcome to British-Genealogy.
First question is - where did they die? If they were shot down over a foreign country then they should appear in the Overseas Deaths index, not the England and Wales GRO Index. Scotland has its own BMD registrations. Ditto the Isle of Man, Jersey, and Guernsey. If they came down in the sea then again they will probably appear in the Overseas Index.
If you'd like to give names and dates, our sleuths will no doubt be able find the answer(s).

As Pam says, you need the Overseas Deaths index. But you don't need the reference, you can just apply to GRO with the information you have.
But why? A death certificate is unlikely to tell you any more than you should already have found in the Commonwealth War Graves site.

Enter Surname and Initials and details of rank, squadron, aircraft, station, date of briefing etc and location of grave/memorial will be returned.
by copying the aircraft serial No. and pasting it into the query box the names of the crew on that flight will be returned.
