I don't know how common this was, but it may be worth mentioning that I've recently discovered that burial board records are included in the minutes books of our local council. They give the name of the person to whom the grave was allocated, usually a relative of the deceased, the cemetery, and plot number. Permission for monuments and inscriptions is also listed. They aren't indexed by name, so you need the approximate date. Council Minutes books are usually found in Local Studies Libraries. As many cemetery offices now have high fees for lookups or may speak only to relatives, this may be a source worth investigating.
