It might be helpful if you tell us whose record it is so we can view the image directly and get more familiar with the handwriting & columns etc, but from the bits I can read:
The first line says he received a gunshot* wound and was admitted to No 26 General Hospital at Etaples
Line 2 - 23/10/15 To Wharncliffe War Hospital, Sheffield 21/10/15
Line 3 I'm not sure of - seeing the full record might help
Line 4 - Wounded in Action 13/10/15
So it seems to be a note of him being wounded, hospitalised, and returned to the UK to hospital in Sheffield.
*GSW is short for Gunshot Wound, but it was also used as shorthand for similar injuries, including shrapnel wounds from shelling etc

First two lines look like:
Gun shot wound shoulder, admitted No 26 General Hospital Etaples 16.10.15
23.10.15 to Wharncliffe War Hospital, Sheffield 21.10.15

I think the second half of the third line may say 'discharged to England'

Yes, i agree, NK 27.10.15 LH 2507 Dis to England 20.10.15
LH 2522 Wd in act 13.10.15 (wounded in action)
I presume the LH and a number are references to some records??
