Looking at a birth reg on the GRO it has
GRO reference DOR Q2/1993 and also 0061A and A71B
Firstly, DOR, I’ve not seen this before, does it mean anything? It’s not the area. And does the A with the numbers mean Adoption? Or merely part of the reg,vol & page?
Didn’t want to put full details as the person is most likely still living.

For comparison, here's an entry copied from the GRO Index on its own website.
DOR Q2/1993 in COLCHESTER (4691B) Reg B59C
DOR is Date Of Registration. Q2/1993 is the usual 2nd quarter (i.e. April to June) 1993.
In later entries I believe the DOR changes to something like 0998 meaning the month of September 1998.
(4691B) is the district number.
Reg B59C is the register number.Vulcan XH558 - “Don't cry because it's over, smile because it happened.”

I had Total brain block! The DOR is obvious now
Is there ever anything on a birth certificate that suggests adoption?

When a court grants the adoption order, the child's original birth register entry will have the word "adopted" noted in the margin, and that will be shown on any certificate issued subsequently, but there will be no indication in the index.
