    Default Mothers name on birth certificate

    Hi
    On my mothers birth certificate in the box for mothers name it says "Jane Ann Watson formerly Smurthwaite" does that mean she was married before or is it her maiden name?

    Thanks for any help.

    "Formerly" means maiden name.

    If she had been married before, the word used is "Late " eg "Late Smith" means she used to be Mrs Smith.

    If there was yet another name involved - an alias - the word to look out for is "otherwise".

    So you could in theory find a name presented as "Jane Smith otherwise Brown late Jones formerly Watkins"

    Note that "maiden name" is not necessarily the same as birth name.
