Hi,
First time with WW1 Army material, a little unsure about items in red
I have the Medal roll index for my grandfather-Ray Wilkins OBE

Could someone have a look at this and explain it to me please.

"Artists R" would be the Artists' Rifles

The London Regiment included the Artists Rifles as one of its battalions, numbered as 28th, in which he was a private. A googling on Artists Rifles will bring you some info. They ran an officer training unit amongst other activities.

The medal roll details are from documents at the National Archives, Kew, not online. They may include more details. As he was commissioned from the Artists Rifles to Royal Army Ordnance Corps, the roll reference is for officers' service ("off").
Officers had to apply for their medals, post-war, other ranks had them sent.
I guess "lsm medal" is for long service, which would be only for other rank service.
